Maker-checker flow. Refunds above $75 require two distinct approvers — or in solo mode: step-up TOTP + 60-minute cool-down.

Maker-Checker Policy ($75 threshold)

Two-Person Mode (production)

Approver must be a different identity from the requester. The engine enforces approved_by != requested_by.

Solo Mode (launch compensating control)

Until a second finance/owner admin exists: step-up TOTP verification + 60-minute cool-down timer + mandatory audit entry. The engine storescooldown_until. Auto-activates real maker-checker on second admin onboard.
